Survivors include: father, David L. Hayes; mother, Alice S. Hayes; daughters, Alyssa Hayes, Samantha Hayes and Brittany Hayes; brothers, Marcus Hayes and Jason Hayes. A memorial service will be conducted Sunday, May 18, 2014 at 2:00 p.m. at Elkin Funeral Service Chapel with Rev. Ivan Dameron and Rev. Adam Collins officiating.
